Bactrocera perfusca can be identified reliably based on morphology. Spot characters are the black postpronotal lobes, black scutellum, all black scutum without yellow markings, and an all black abdomen.

Bactrocera perfusca is only known from the Marquesas Islands in the South Pacific, which is its presumed native distribution.
